Accepting new patients, including new Medicare, Medicaid, private insurance and self-pay. A referral is not required, but is recommended for all new patient appointments to ensure the patient is seeing the correct specialist for the correct condition.

Overview
Our geriatrician works with a patient’s primary care provider to offer cognitive evaluations for older adults, comprehensive geriatric assessments - including frailty, falls, multimorbidity goals of care and polypharmacy (multiple medications), and consultations for cognitive impairment / dementia. For patients with advanced dementia or other geriatric syndromes, geriatric primary care is also available.

This clinic is a department of Community Medical Center. When you see a physician or receive services in a provider-based clinic, you are technically being treated in an outpatient department of the hospital rather than the physician’s office.
