- Patients must be homebound and physically unable to come into the clinic for visits
- Seniors living in a residential community or assisted living facility

Home Based Primary Care Providers
Overview
Home Based Primary Care is a convenient service that allows homebound adults that are age 65+ with complex or chronic conditions to establish care with a primary care provider. Patients can receive personalized primary care in their home, assisted living and residential communities through in person and telehealth visits. Our adult medicine team focus on taking care of the whole person with access to a behavioral health social worker and care manager over the phone. These services are covered as a provider visit through Medicare Part B.

This clinic is a department of Community Medical Center. When you see a physician or receive services in a provider-based clinic, you are technically being treated in an outpatient department of the hospital rather than the physician’s office.
